MIC Project Request Form

This form should be utilized to request a video project from the Multimedia Innovation Center. Please complete this form in its entirety. All form submissions will be sent directly to Liz Keptner.

Please note that based on volume and priorities, not all requests will be accommodated. You will be contacted by the MIC to set up a planning meeting, or notified if the MIC is unable to accommodate this request.
